This Privacy Policy explains how JB Behavioral Health, LLC, collects, uses and discloses personal information of its clients, prospective clients, and visitors to its website at:  https://www.jbbehavioralhealth.org/contact 

COLLECTION OF PERSONAL INFORMATION: We may collect personal information directly from prospective clients over the phone, in person, via email or through a web form on our website https://www.jbbehavioralhealth.org/contact 

Personal information collected includes first and last name, email and phone number.  The information received is stored in the Electronic Health Record and shared with the Administrative Team including the Client Service Representative, Billing Manager and Clinic Director.  Treating providers also have access to phone numbers for their assigned clients.  Providers do not have access to other clients who are treated by other providers unless they perform in the supervision role of other providers employed or contracted by JB Behavioral Health, LLC. 

  

DISCLOSURE OF PERSONAL INFORMATION: SMS opt-in of phone numbers for the purposes of SMS are not shared with any third party and affiliate companies for marketing purposes.

TYPES OF SMS COMMUNICATIONS:   If you consent to receive SMS messages from JB Behavioral Health, you agree to messages related to scheduling / rescheduling appointments, appointment reminders and documentation or account completion reminders SMS from us.

Example: Hello [Client First Name]. We are confirming the change in your appointment with [Provider first name] from [date/time] to [date/time]. We look forward to seeing you then!  Reply START to opt-in to receive SMS Messages from JB Behavioral Health. Reply HELP for assistance or visit our website to see our privacy policy and terms of conditions. Reply STOP to opt-out of receiving SMS messages from JB Behavioral Health. MESSAGE FREQUENCY: JB Behavioral Health’s SMS message frequency is minimal and will be approximately 5-10 SMS messages daily across all users.  

POTENTIAL FEES FOR SMS MESSAGING: Many carriers charge a fee for each message sent or received.  This can vary depending on the carrier’s pricing structure and whether the messages are sent domestically or internationally.  Users are encouraged to contact their respective carriers for any possible associated fees.

OPT-OUT METHOD: Clients will be able to opt-out of SMS messaging from JB Behavioral Health by replying STOP at any time to any received SMS message.  Once opted out, the client will no longer receive further SMS communication.  Client may opt back in to SMS messaging at any time by replying START.
